Sharing Information Consent and Privacy Rights Guide.
Wild Hearts Coordination and Supports (WHCAS) collects, uses, and stores Personal Information in accordance with our Privacy & Confidentiality, and our Information Management Policy and Procedures. Our policies meet all legal and ethical requirements. Information is only used for purposes that are reasonably necessary to provide informed, holistic supports that provide our participants personalised, safe, and effective services. The information we collect meets our legal and funding requirements.
It is important that you understand your privacy rights before you provide us with written consent to collect and share your information. You (or your legal guardian) will need to provide consent regarding information collection and sharing while engaging WHCAS to provide NDIS services.
What Information Do We Collect and Share About You?
WHCAS may collect and store information about you which we consider is required in order for us to provide our services to you. We may share this information with other providers, allied and medical practitioners.
What Are My Rights?
WHCAS is committed to protect your information in accordance with the Australian Commonwealth Privacy Act 1988, Queensland Privacy Act 2009 and the NDIS Act 2013. Our Privacy and Confidentiality Policy and Procedures align with all legislation and standards. You have the right to request access to information that WHCAS collects about you and WHCAS encourages you to request the correction of records if you believe your Personal Information is incorrect, incomplete, out of date or misleading.
If there is ever an instance where we may not be required or able to provide you with access to your personal information, we will provide you with the reasons for our decision in accordance with the Commonwealth and State Privacy Acts.
Should you disagree with our decision you can contact the NDIS and also tQueensland Health Ombudsman who receive and investigate complaints about health services and health service providers, including registered and unregistered health practitioners.
Sharing Information with Others.
WHCAS may share your information with necessary staff members, related entities and third parties who assist us in providing services to you, or in operating our business. You can nominate the providers and individual people you give consent for WHCAS to share your information with, at any time. The first time you will be required to decide who you consent to us sharing your information will be during your intake. Afterwards you can update your consents by contacting WHCAS Directors to promptly action your request.
You can also indicate who you don’t want your information shared with. Your participation in our service may require sharing of information with certain people and providers and if you do not consent to any disclosure then we may not be able to provide our services to you. When completing our Permission to Share Information Form WHCAS staff can advise you on this and provide the opportunity for you to ask questions.
All Personal Information is stored in Australia.
The services we have listed for nomination are The Office of the Public Guardian, The Public Trustee of QLD, National Disability Insurance Agency (NDIA), National Injury Insurance Scheme (NIIS), Queensland Health, Queensland Corrective Services, Department of Communities, Child Safety and Disability Services QLD, Department of Justice and Attorney General, Department of Social Services, Department of Education and Training, GP/GP Practice, Specialist Medical Practitioners, Allied Health Practitioners, Family/Carer(s), and other NDIS Funded Services.
In some specific situations we may be required to share your information without your consent. Wherever reasonable you would be notified of how and what information is shared about you. These specific situations include where disclosure is required by law; where your health or safety is at serious risk; and where the health and safety of others is at serious risk as determined by us in our discretion.
We may use your Personal Information to send you information about our services and events that may be of interest to you. Your information will not be provided to third parties for direct marketing purposes.
Where Can I Learn More About My Privacy Rights?
Please speak to a WHCAS staff member if you have any questions regarding information sharing and privacy. Our Privacy Policy, which can be accessed on request, sets out how you can access and ask for correction of your Personal Information, how you can complain about privacy related matters and how we respond to complaints. Withdrawing Consent
You can withdraw your consent at anytime by notifying a WHCAS Director by phone or email. If you withdraw your consent, your personal information will not be shared as per your request or used in any promotional activities and publications produced following the date that the withdrawal request is received by WHCAS.
